The Killer Disease: Smallpox

In the 1700s, smallpox was one of the most feared diseases in Britain. It was highly infectious, caused a fever and a spreading rash of blisters, and it killed a large share of those who caught it. Survivors were often left badly scarred or blinded.

Because smallpox was so deadly, people were desperate for any way to avoid it. This desperation set the scene for one of the most important breakthroughs in the whole Health study: the move from treating disease to preventing it.

This is a turning point in the theme of the course. Earlier, medicine had focused on trying (and usually failing) to cure the sick. Jenner's work was about stopping people getting ill in the first place — the beginning of preventive medicine.

Understanding how bad smallpox was helps explain both why people accepted the risky method that came before Jenner, and why his safer method mattered so much.

Inoculation: A Risky Head Start

Before Jenner, the main defence against smallpox was inoculation. This meant deliberately giving a healthy person a small dose of smallpox itself, in the hope they would suffer a mild case and then be immune.

Inoculation was made popular in Britain by Lady Mary Wortley Montagu, who had seen it used in the Ottoman Empire (Turkey) and promoted it in the early 1700s.

The problem with inoculation:

  • It used real smallpox, so patients could catch a serious, even fatal case.
  • Those inoculated could spread smallpox to others while ill.
  • It was expensive, so mainly the wealthy could afford it, and doctors made money from it.

So inoculation showed that prevention was possible, but it was too dangerous to be a real solution. That gap is what Jenner filled.

Edward Jenner's Breakthrough, 1796

Edward Jenner (1749–1823) was a country doctor in Gloucestershire (and a former student of John Hunter). He noticed something local people already suspected: milkmaids who had caught cowpox — a mild disease from cows — did not seem to catch smallpox.

Jenner decided to test this idea using careful observation and experiment:

  1. In 1796, he took pus from a cowpox sore on a milkmaid, Sarah Nelmes.
  2. He put it into cuts on the arm of a boy, James Phipps, who then caught mild cowpox.
  3. Later, he deliberately exposed Phipps to smallpox.
  4. Phipps did not catch smallpox — the cowpox had protected him.

Jenner repeated his test on other patients, then published his findings in 1798. He named the method vaccination, from vacca, the Latin word for cow.

Jenner's use of observation, testing and repetition makes him a classic example of the factors of the individual and science — and also of chance, since the link with milkmaids was a lucky observation.

Opposition to Vaccination

Even though vaccination clearly worked, Jenner faced strong opposition. This is a favourite topic for the exam because it shows that progress is rarely smooth.

Cartoons of the time mocked vaccination, some even suggesting that patients would grow cow-like features. This resistance meant vaccination spread more slowly than it might have.

Government Action and Jenner's Significance

What finally helped vaccination win out was the growing role of government — an important shift, because for centuries governments had done almost nothing about health.

  • 1802 and 1807: Parliament granted Jenner money to continue his work, a sign of official support.
  • 1840: an Act made vaccination free for the poor and banned the older, riskier inoculation.
  • 1853: an Act made smallpox vaccination compulsory for infants, though enforcement in practice was often weak.

Jenner's long-term significance is enormous:

  • He introduced the first successful vaccine, and the principle of vaccination was later used against many other diseases.
  • Smallpox was eventually wiped out worldwide in the 20th century, following the path Jenner began.

Jenner did not understand germs, so he could not develop vaccines for other diseases himself. That step waited for Louis Pasteur and germ theory later in the 1800s. But Jenner's method was the vital first move in preventive medicine.

He is therefore a superb case study for weighing up factors: the individual (Jenner), science (his method), chance (the milkmaid observation) and government (the Vaccination Acts) all combined to spread his discovery.

Common Exam Mistakes

1. Confusing inoculation and vaccination

Inoculation used a dose of smallpox itself and was dangerous. Vaccination, Jenner's method from 1796, used the mild cowpox and was much safer. Do not use the words as if they mean the same thing.

2. Getting the dates wrong

Jenner carried out his first vaccination in 1796 and published his findings in 1798. The key Vaccination Acts were 1840 (free for the poor) and 1853 (compulsory for infants).

3. Saying Jenner understood germs

He did not. Jenner could prove vaccination worked but could not explain why, because germ theory came decades later with Pasteur. This is exactly why he faced so much doubt.

4. Overstating the immediate impact

Vaccination protected against smallpox alone, and opposition slowed its spread. It did not immediately cure other diseases. Balanced answers explain both its huge long-term significance and its limits at the time.

5. Forgetting the factors in the essay

Jenner links several factors: the individual, science, chance and government. In the 16-mark factors essay, naming which factor each fact illustrates is what lifts an answer into the higher levels.
