The Public-Health Crisis of the Industrial Towns

In the first half of the nineteenth century, Britain's population poured into fast-growing industrial cities like Manchester, Leeds and London to work in the new factories. Housing could not keep up.

Workers were crammed into cheap, back-to-back slums with terrible sanitation:

  • No clean water supply — families shared a single pump, often near a cesspit.
  • No proper sewers — human waste ran in the streets or seeped into wells.
  • No rubbish collection — filth piled up in courtyards.
  • Overcrowding — whole families shared a single damp room.

These conditions were perfect for disease. Typhoid, tuberculosis and above all cholera spread through the slums. Cholera epidemics struck Britain in 1831–32, 1848–49, 1854 and 1866, killing tens of thousands each time.

At the time, most people blamed miasma — bad air and smells — for these epidemics. No one yet understood that the real killer was often the water supply.

John Snow and the Broad Street Pump, 1854

The breakthrough came from a London doctor, John Snow, who doubted the miasma theory. He suspected cholera was spread by something people swallowed, most likely contaminated water.

During the 1854 cholera outbreak in Soho, London, Snow tested his idea. He mapped every cholera death in the area and found they clustered tightly around one water source: the Broad Street pump. People who drank from other pumps were far less likely to fall ill.

To prove his point, Snow removed the handle of the Broad Street pump so no one could use it. Cholera deaths in the area fell. He had shown that cholera spread through dirty water, not bad air.

There was a limit to Snow's work: he proved how cholera was transmitted, but he could not explain why dirty water caused disease, because Germ Theory did not exist yet (Pasteur published in 1861). Many officials therefore remained unconvinced for years.

The Reformers and the Barrier of Laissez-Faire

Change was slow because of a powerful belief called laissez-faire — the idea that government should not interfere in people's lives, health or businesses. Many politicians thought cleaning up the towns was not the state's job, and ratepayers did not want to pay for it.

Against this stood reformers who argued that dirt, disease and poverty were linked.

  • Edwin Chadwick published a major Report on the Sanitary Condition of the Labouring Population in 1842. He argued that disease was caused by filthy living conditions and that improving them would cut death rates — and save money, because fewer sick workers meant fewer people needing poor relief.
  • Chadwick's report and the shock of repeated cholera epidemics slowly built pressure for the government to act.

Chadwick's key argument was practical: a healthier population was a cheaper population. Preventing disease would reduce the cost of supporting the sick and orphaned poor.

The Great Stink and Bazalgette's Sewers

In the hot summer of 1858, the River Thames, full of London's raw sewage, gave off such a stench that Parliament — which sits beside the river — could barely function. This event became known as the Great Stink.

Politicians who had ignored the slums could no longer ignore the smell under their own windows. Parliament finally agreed to fund a solution.

The engineer Joseph Bazalgette was commissioned to build a vast new sewer system for London. Through the 1860s he constructed around 1,300 miles of sewers that carried waste away from the city and downstream, well clear of the water supply. It was one of the greatest engineering projects of the century.

The Great Stink is a classic example of the role of chance: the reform came not from careful planning but because the smell happened to reach Parliament. It shows how a single event can force a reluctant government to act.

Bazalgette's sewers dramatically reduced cholera in London, even before Germ Theory explained exactly why clean water saved lives.

The 1848 Act was passed during a cholera epidemic but was permissive: it only allowed local action rather than requiring it. Because of laissez-faire attitudes and cost, most towns ignored it.

By 1875 attitudes had shifted. Repeated epidemics, the reformers' arguments, and the 1867 extension of the vote to many working-class men (who wanted better conditions) pushed the government further. The 1875 Public Health Act was compulsory: every local authority now had to provide clean water, proper drainage and sewerage, and appoint health officials.

This marked a decisive change: the government now accepted responsibility for public health, ending the era of pure laissez-faire.

Common Exam Mistakes

1. Saying Snow discovered the cause of cholera

Snow proved how cholera was transmitted — through dirty water. He did not discover its cause, because the cholera microbe was unknown until Germ Theory (Pasteur 1861, Koch identified the cholera bacterium in 1883). The examiner's report specifically warns against this confusion.

2. Confusing the 1848 and 1875 Acts

The 1848 Act was voluntary and largely ineffective; the 1875 Act was compulsory and transformative. Getting these the wrong way round undermines your point about government's changing role.

3. Forgetting laissez-faire

Reform was slow mainly because governments believed they should not interfere. Naming laissez-faire explains why it took decades to clean up the towns, earning higher marks than simply describing the slums.

4. Ignoring the Great Stink's significance

The Great Stink of 1858 was not just a bad smell. It was the trigger that finally forced Parliament to fund Bazalgette's sewers — a clear example of the role of chance in reform.

5. Describing filth without explaining consequences

Do not just list how dirty the towns were. Explain how overcrowding and dirty water caused epidemics, and how that eventually led to government action. The exam rewards cause and consequence, not description.
